Register with the practice as a new patient

How to register

You can register with us if you live inside our catchment. Our current catchment is shown in blue on the map below. Our current surgery sites are also shown in (red dots). If you would like to register with us online, use the link below and it will take you to a webpage where you can register online. The webpage will ask you for your postcode so we can check you live in our catchment. It will also ask you for some information to help us register you with our services. Once you have submitted your application to register, we will assign you with a surgery near to where you live and allocate you a GP at that surgery site to oversee your care -this will be your named GP. Our surgery sites are listed here. You can also register with us by coming to one of our reception desks and speaking with one of our team.

You will not be refused registration or appointments because you don’t have a proof of address or personal identification at hand

This also applies if you are an asylum seeker, refugee, a homeless patient or an overseas visitor, whether lawfully in the UK or not.

Student registration 

You are able to register with Southampton Sea City Partnershipy using our online registration form. You can do this before you arrive once you know the details of your new address. You will be asked to confirm proof of identity and date of birth and we may contact you by phone to verify this. Please ensure that you attach your photo ID with your registration. We will use these details to check it matches with the information held on the NHS central patient registry and that your previous medical notes are passed on to the practice.

You will also be able to do this in person during your first week at University as staff from Southampton Sea City Partnership will be on site at the University to assist you.

When completing the online forms, you will also fill out a medical questionnaire. This is needed as it can take a considerable amount of time for us to receive your medical records.

On completion of the above form, you will be registered at Southampton Sea City Partnership. Please browse our website to familiarise yourself with services we offer and how you can access them.

Please remember to tell us whenever you change your mobile phone number and let us know if you are happy for us to contact you by text message.

Tips for completing the form

  • If you do not know your NHS number, please leave it blank.
  • Your 'home address' is your new address in Southampton.
  • Your 'previous address' is the address where you have just moved from to come to Southampton

Temporary services

If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.

To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

Continuity of care and Registered GPs

At Southampton Sea City Partnership we are keen to promote continuity of care wherever possible.  This is particularly relevant to patients with more complex or long-term conditions and means that we will try to ensure that you deal with the same doctor (and/or other members of our team).  This helps build trust and also helps you and our clinicians to get to know one another so we can better understand your choices and priorities.   There is good evidence that continuity of care can help improve clinical outcomes and patient experience.

When you register at Sea City Partnership, we will allocate you a “Registered GP” and will tell you who this is.  Your Registered GP will take a lead role in the coordination of your care. This is particularly relevant if you have more complex and long-term health needs, in which case you can expect to have appointments and/or talk with your Registered GP periodically to discuss your care.  The role of the Registered GP will be more significant for patients with more complex needs.  Your Registered GP will usually take the lead responsibility for other aspects of your care including interpreting diagnostic test results (e.g. blood results) and liaising with other clinicians and services about your care.   Your registered GP will also be supported by other members of our team which means that sometimes you will be seen by other doctors, advanced practitioners, pharmacists, or other members of the team who will be well placed to meet your needs.  We have good communication across our team, and your Registered GP will coordinate and link with these other team members to ensure your care is joined up.

Sometimes where your care needs are less complex or more short-term, you will more likely be offered care from other members of the team, to enable you to be seen more quickly and to ensure that your needs are best matched with someone from our team that can help.
